Privacy Policy for Clinical Athlete's physical therapy pages

Clinical Athlete LLC runs clinicalathlete.com. This policy covers the site’s physical therapy pages and the booking form on them. Clinical Athlete’s education business has its own privacy policy.
Booking and patient information
What you give on the booking form (your name, email address, mobile number, state, and your text and email choices) goes to the Slate Physical Therapy practice for your state. It is handled as health information under the practices’ Notice of Privacy Practices, which explains how they use and share it, and your rights. They use it to arrange your call and, only if you ticked the box, to email you news and offers. It is shared only with the practices’ management company and the companies that run the practices’ booking, scheduling and messaging systems, under contracts that require them to protect it. It is never sold, and your mobile number and texting choice are never shared with anyone for their marketing.
Information collected automatically
When you visit these pages, your browser sends information such as your device and browser type, IP address and the pages you visit. We use cookies and similar tools so the pages work and so we can see, in total, how they are used. Some are run by other companies, which may collect information about your visits over time and across websites. These pages do not respond to “Do Not Track” signals.
