Financial Policy

Slate Physical Therapy, P.A. · Slate Physical Therapy East, P.C. · Slate Physical Therapy West, P.C.

Version of October 19, 2026

This policy sets out how you pay for physical therapy from the Slate Physical Therapy practice that treats you, which depends on the state you are in during your sessions: Slate Physical Therapy, P.A. (all states except California, New Jersey and New York), Slate Physical Therapy East, P.C. (New Jersey and New York) or Slate Physical Therapy West, P.C. (California). It is part of the care agreement you sign with that practice, and the version you agreed to is the one that applies to you. The terms are the same for all three practices, except where a box below says otherwise.

At a glance

Your care

Physical therapy by telehealth, in 14-day periods: your program, messaging with your therapist and any calls in that period. It is not a fee per visit.

Your first payment

At checkout, before your evaluation: $225 if you start in the Rehab Phase, or $175 if you start in Clinical Performance. It covers your evaluation and your first 14 days of care, counted from the day of your evaluation.

After that

The price of your care phase every 14 days, charged automatically until you stop: $225 in the Rehab Phase, $175 in Clinical Performance. A change of phase starts on a charge date, only once you agree to it.

Minimum term

None. You can stop at any time.

How to stop

Use "Stop my care plan", a short form linked in your portal and in every billing email, or message us in the Relay app, email slate@clinicalathlete.com or tell your therapist. A charge not yet made when we receive your request is not made.

Your evaluation

Reschedule at no charge. Cancel at least 24 hours before it for a full refund. If you cancel later or miss it, there is no refund, but you can rebook once within 30 days.

If your therapist refers you elsewhere at your evaluation

Your first payment is refunded in full.

If a payment fails

We tell you and try again. Care continues for 7 days, then new programming pauses until your card is updated, with no new charges while paused. After 30 days unpaid, your care ends, with notice and help finding other care.

Clinical Strategy Consult

$395, once. If you book ongoing care within 30 days after it, your consult fee includes your first payment.

If your care is with Slate Physical Therapy East, P.C. (New Jersey and New York)

Nothing is charged before your evaluation. Your evaluation costs $225, paid after it, unless your therapist refers you elsewhere at the evaluation. If you continue, that $225 is your first payment and also covers your first 14 days of care, counted from the day you pay; your next charge is 14 days after that payment. If you don't continue, we email you a one-time payment request for the $225. Rescheduling and cancelling your evaluation are free, and if you miss it you can rebook once at no charge.

Who takes your payments

Slate Physical Therapy, P.A. takes the payments for its own patients. It also takes payments for Slate Physical Therapy East, P.C. and Slate Physical Therapy West, P.C. as their billing agent, a company that takes payments for another under a written agreement, and paying the P.A. pays the practice that treats you. Your agreement, for your care and for your payments, is with the practice that treats you. Your card statement shows these charges as {CARD DESCRIPTOR}.

Your first payment, and when your 14 days start

Your first payment is taken at checkout, before your evaluation: $225 if you start in the Rehab Phase, or $175 if you start in Clinical Performance. It covers the set-up before your evaluation, including your therapist’s review of your forms; your evaluation by live video; and your first 14 days of care, counted from the day of your evaluation. Your next charge is 14 days after your evaluation, and then every 14 days. If your evaluation moves, your next charge moves with it, and we email you the date. Each later charge pays for the 14 days that start on its charge date. A charge covers everything in its 14 days (your program, messaging with your therapist and any calls in that period); it is not a fee per visit.

If your care is with Slate Physical Therapy East, P.C.

Your first payment is the $225 for your evaluation, paid after it. If you continue, it also covers your first 14 days of care, counted from the day you pay. Your next charge is 14 days after that payment, at the price of your care phase, and then every 14 days.

Charges every 14 days, and your care phase

Your therapist recommends the phase that fits your care. A change of phase, in either direction, starts on the first charge date at least one business day after you agree to it: a reply to our email or a message in the Relay app is enough. Until you agree, your care plan stays as it is, and you can stop at any time. If your Good Faith Estimate doesn't already cover the change, we send you a new one before it starts.

If we change our prices, we tell you in writing 14 to 28 days before your new price applies, and how to cancel. If your price goes up, you may cancel within 14 days after the first charge at the higher price, and we refund the unused part of that 14-day period, counted by days. This is a pro-rated refund: for example, if you cancel with 9 days of the period left, you get back 9/14 of that charge. It is an exception to the rule that a period that has started is not refunded.

There is no minimum term. Your charges continue every 14 days until you stop them.

How you pay

If your care is with Slate Physical Therapy East, P.C.

Your charges go to the card you enter at checkout, which our payment processor keeps on file for them. That's how our care plans run. It is not a condition of your care: if you'd rather not keep a card on file, tell us at any time, before or after checkout, by replying to our email, messaging us or telling your therapist, and we'll send you a payment link before each 14-day period instead, at the same price. If you pay by credit card, what you owe the card issuer becomes credit-card debt rather than a medical bill owed to us, and you may lose state and federal protections for medical debt, including limits on interest, credit reporting, property liens and wage garnishment.

Stopping your care

You can stop at any time, without giving a reason. Use "Stop my care plan", a short form linked in your portal and in every billing email, or message us in the Relay app, email slate@clinicalathlete.com or tell your therapist.

Your request counts when we receive it: a charge not yet made is not made, and we refund any charge made after we received it. We confirm by email the same business day, or the next business day if your request reaches us on a weekend or holiday. Your care continues to the end of the 14 days you paid for. A period that has started is not refunded, except as listed under "Refunds" or as the law requires.

To start again later, contact us. After a gap of more than 30 days, care restarts with a new evaluation and a new first payment.

Your evaluation: rescheduling, cancelling and missing it

If your care is with Slate Physical Therapy East, P.C.

Rescheduling and cancelling your evaluation are free. If you miss it, you can rebook once at no charge. There is no fee for a late cancellation or a missed evaluation, and nothing is charged if your therapist refers you elsewhere at it.

Calls during ongoing care

If you need to move a call, please tell us at least 24 hours ahead. There is no fee for a missed or moved call.

Refunds

We refund:

Refunds go back to the card you paid with, within 30 days of the cancellation, referral or confirmation that triggers them. The Clinical Strategy Consult has its own refund terms, below.

If a payment fails

If a charge does not go through, we tell you and try again. Your care continues for 7 days after the failed charge. After that, your therapist pauses new programming until you update your card; you can still message us, and your therapist still answers any concern about your safety. No new charges are made while your programming is paused. When you update your card, we charge the payment that failed, your programming restarts, and your next charge is 14 days later. If the payment is still unpaid 30 days after it failed, your care ends, as described under "If we end your care". The unpaid payment stays owed. We do not add interest, late fees or collection costs.

If we end your care

If we end your care for any reason other than your own decision to stop, for example after 30 days unpaid, we:

If you move to another state

Tell us before you move to another state, or as soon as you've moved. The period you have paid for stays paid. From your next 14-day period, your care is provided, and charged, by the Slate Physical Therapy practice for your new state, which asks you to sign its own forms first. If none of our therapists can treat you in your new state, we end your care as described above and refund the unused days of your paid period.

Clinical Strategy Consult

A Clinical Strategy Consult is one live video session with a physical therapist of the practice for your state about a specific question, decision or plan, with review beforehand of materials you send us. It does not include ongoing care, a program, app access or messaging afterward. You may ask for a written summary of your consult.

Insurance, superbills and Medicare

Insurance may not cover physical therapy, and services without a referral might not be covered by your health plan or insurer.

None of the Slate Physical Therapy practices is in any health plan's network: they have no contract with any plan, do not take payment from your plan, and do not send claims to your plan for you. You pay us directly.

On request, we give you a superbill, free of charge: an itemized statement you can send to your health plan yourself. Any superbill will report only services actually rendered and will disclose the portion of your bundled payment prospectively allocated to those services. Program services that lack an applicable billing code are not represented as insurance-covered services, and reimbursement is not guaranteed. In plain words: a superbill lists only the evaluation and calls your therapist actually gave and documented, with the share of your payment we set in advance for each one; the rest of your program is not listed as covered care; and whether your plan pays you back is up to your plan.

We do not issue a superbill for Clinical Performance periods, which are a wellness service, or for a Clinical Strategy Consult unless your therapist gave and documented an evaluation or treatment during it; you get a receipt for those. Fees, such as a missed-appointment fee, never appear on a superbill.

Medicare. None of the Slate Physical Therapy practices is enrolled in Medicare or bills Medicare. For now, we can't start care for anyone with Medicare Part B, including everyone in a Medicare Advantage plan. We'll add you to our waitlist, and a Medicare-enrolled physical therapist can treat you in the meantime. If you have Medicare Part A only and another plan is your primary coverage, you can start care with us. If you enroll in Part B during your care, tell us as soon as you know your start date: your care ends the day before it starts, and we refund any unused days you've paid for.

Estimates of your charges

You have the right to receive a Good Faith Estimate of your expected charges. We give every patient one in writing within one business day after you book your evaluation, and at any time you ask. Your rights: Good Faith Estimate notice · En español. An estimate is not a contract; this policy and your care agreement hold the terms.

Your records never depend on payment

We never hold back your records, or a copy of them, because of an unpaid charge.
