Your right to a Good Faith Estimate
Under federal law, health care providers must give people who do not have health coverage, or who are not using it for their care, a written estimate of expected charges. Each Slate Physical Therapy practice gives one to every patient, whether or not you have coverage.
- When you book your evaluation, we send your written estimate within one business day.
- You can also ask for an estimate at any time: on your discovery call, by email to slate@clinicalathlete.com or by calling 667-239-5010. We send it in writing within 2 business days.
- If you have no health coverage, or you are not having a claim for this care sent to your coverage, and your bill is at least $400 more than your estimate, you may be able to dispute the bill.
- Keep a copy of your estimate.
- For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises/consumers or call 1-800-985-3059.

Your rights and protections against surprise medical bills in New York
This notice is from Slate Physical Therapy East, P.C., which treats patients in New York.
What is balance billing? When you see a provider who is not in your health plan’s network (“out of network”), the provider may bill you for the difference between what your plan pays and the provider’s full charge. This is called balance billing. A surprise bill is a balance bill you did not expect.
When New York law protects you. Under article 6 of New York's Financial Services Law, if your health plan is regulated by New York, you owe only your usual in-network share (your copayment, coinsurance or deductible) for:
- emergency services;
- services from an out-of-network provider at an in-network hospital or ambulatory surgical center, when an in-network provider was not available or unforeseen services were needed;
- services from an out-of-network provider that an in-network doctor referred you to, in the situations the law lists, unless you signed a written acknowledgment that the referral may result in costs your plan does not cover.
If you have no health plan, New York also protects you from some bills from doctors at hospitals and ambulatory surgical centers when you were not given the required information about charges. Self-funded employer plans follow the federal rules instead.
Federal protections. The federal No Surprises Act also stops out-of-network providers from balance billing you for emergency services, and for most non-emergency services at in-network hospitals and certain other facilities unless you agreed in writing in advance.
How this applies to our care. Slate Physical Therapy East, P.C. provides scheduled physical therapy by telehealth. It does not provide emergency services and does not treat patients at hospitals or ambulatory surgical centers. It is out of network for every health plan and does not bill health plans: you pay it directly, at the prices on our Good Faith Estimate page, and you receive a written Good Faith Estimate.
If you think you have been wrongly billed, contact the New York State Department of Financial Services at (800) 342-3736, surprisemedicalbills@dfs.ny.gov or www.dfs.ny.gov. For the federal protections, visit www.cms.gov/nosurprises or call 1-800-985-3059.
